Honestly, he would benefit from therapy, but that would be unlikely because they live in such a remote area,

That was another thing I liked that Jake's tragic backstory makes sense within the setting, Sure, it could have worked if the book was set in a city, but the tragedy is all the more disturbing for its utter believability in an extremely rural area without good access to medicine.
